We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our clinical team for an 11-week contract. This role is ideal for an LPN who thrives in a supportive, patient-centered environment and enjoys providing hands-on care in a rehabilitation setting.
You’ll work closely with other healthcare professionals to deliver exceptional care to residents while maintaining high standards of safety, documentation, and professionalism.
Provide direct nursing care in accordance with the facility’s policies and state regulations
Administer medications and treatments accurately and on schedule
Monitor and document residents’ conditions and report any changes to the supervising RN or physician
Assist with daily living activities and ensure residents’ comfort and safety
Collaborate with interdisciplinary care teams to develop and implement care plans
Maintain accurate records in EMR systems and uphold confidentiality standards
Support infection control and quality assurance initiatives
Active LPN license in the state of employment
Current CPR/BLS certification
Previous experience in long-term care, skilled nursing, or rehabilitation preferred
Strong communication, time management, and teamwork skills
Reliable and professional with a strong commitment to patient care
